At 10:00 a.m. on June 8, 2018, an employee was operating an order picker in a cold storage warehouse. The employee did not attach lanyard to harness when elevated 20 feet above ground on a stock picker. A coworker in the same aisle operating a forklift hit the order picker, causing the employee to fall to his death.
