At 11:00 p.m. on March 28, 2018, an employee was troubleshooting the Mixer Six table that was vibrating and put his finger into the area where the rotating blades of the equipment were operating. The employee struck two of his fingers with the blades and amputated the tips of his right index and middle fingers. The employee was hospitalized.
