At 9:15 a.m. on January 19, 2018, three employees were taking a break in an upper engine room near an opening that was covered with plastic and they discovered that the atmosphere had been contaminated with oxygen and propylene fumes that had bled out from their cutting torch tanks into the vessels forward portable water tank. The employees had turned on two ventilation fans to remove what gas had bled out from the tanks and vented the space for over 90 minutes before resuming work. Three employees suffered internal injuries as well as burns when their tanks ruptured and exploded in a confined space. Two employees were hospitalized with the third employee treated and released. | ||||||
