At 2:45 a.m. on December 25, 2017, Employee #1, a Paper Winder Assistant Operator, was trying to clear a paper jam that had occurred between a knife vane and two paper winding rollers in a paper winder machine. The employee went under the paper winder rollers to remove the jam. The knife vane dropped onto the employee, causing a deep laceration to his left lower arm, wrist and hand, resulting in an amputation. Employee #1 was hospitalized for his injury.
