Accident Investigation Summary
Summary Nr: 101211.015 Event: 11/28/2017Employee Is Killed When His Head Is Caught Between Tailgate
At 11:00 a.m. on November 28, 2017, Employee #1 was tasked to repair the tailgat e door of a garbage truck, located at the rear section of the truck. The tailgat e door of the Mack front loader garbage truck, Vehicle Identification Number: 1M 2K195C54M025743, was approximately 95 feet wide by 100 feet high. Employee #1 pl aced and rested the tailgate door on top of the mast of a Toyota powered industr ial truck (PIT), Serial Number: D738330. The top of the mast was approximately 93 feet above the ground. The tailgate door weighed approximately 600 pounds. Em ployee #1 was located between the tailgate door and the garbage truck hopper. Th e hopper was the section of the truck where debris was dumped when collecting ga rbage. The hopper opening was approximately 92 feet wide by 82 feet high. The ho pper opening was approximately 52 feet above the ground. A mechanic was located on the PIT. Another coworker was replacing parts in the cab of a Ford aerial bo om lift truck, Vehicle Identification Number: 1FDAF56FXXED11844. The truck was positioned at the right rear of the garbage truck. The mechanic needed to use the PIT to move equipment inside the third bay of the garage. The mechanic rais ed the mast and backed the PIT. The tailgate fell and caught Employee #1's head between the tailgate door and the lower section of the hopper. Employee #1 rece ived skull fractures and was killed.
