At 9:30 a.m. on October 4, 2017, an employee was descending a newly constructed industrial stairway approximately 90% complete after his crew was done erecting a scaffold in the GDU (gasoline desulfurization unit) unit at a plant. The employee was found on the 4th level but above him approximately 19 feet above on the 5th level was an opening approximately 3 feet by 3 feet. His hands were resting on the mid-rails. His body was facing towards the landing with his feet towards the stairs facing east. No witnesses saw the employee fall. Witnesses who responded to him found him initially conscious but not talking with no visible external trauma. A few minutes later he went unconscious. CPR was started on him, and an AED was placed but not used prior to his transport to the hospital. No trauma was found by ER doctor treating the employee and was ruled by ER doctor to be a heart attack. | ||||||
