Accident Investigation Summary
Summary Nr: 100183.015Event: 07/10/2017Employee Slips On Floor And Sustains Head Injury And Chemica
At 3:10 p.m. on July 10, 2017, Employee #1, a school custodian, was using a floor wax stripper (Maintex Wax Eliminator- hazardous ingredients: 2-Butoxyethanol, Potassium Hydroxide, and 1-Amino-2-propanol) on the floor of the nurse's office. A coworker spoke to the employee through an open door behind the employee. As the employee turned towards the coworker, he slipped on the wax stripper that was on the floor, falling onto his back, hitting his head and losing conscientiousness. The coworker came to his aid and did not move him for fear of head/neck injury. Employee #1 laid in the wax stripper until emergency services arrived. The employee sustained chemical burns to abdomen, back, legs, arms, over approximately 19% of total body surface. The employee was taken to the hospital where he was treated and admitted to the burn unit.
