Accident Investigation Summary
Summary Nr: 100183.015 Event: 07/10/2017Employee Slips On Floor And Sustains Head Injury And Chemica
At 3:10 p.m. on July 10, 2017, Employee #1, a school custodian, was using a floo r wax stripper (Maintex Wax Eliminator- hazardous ingredients: 2-Butoxyethanol, Potassium Hydroxide, and 1-Amino-2-propanol) on the floor of the nurse's office. A coworker spoke to the employee through an open door behind the employee. As the employee turned towards the coworker, he slipped on the wax stripper that wa s on the floor, falling onto his back, hitting his head and losing conscientious ness. The coworker came to his aid and did not move him for fear of head/neck in jury. Employee #1 laid in the wax stripper until emergency services arrived. The employee sustained chemical burns to abdomen, back, legs, arms, over approximat ely 19% of total body surface. The employee was taken to the hospital where he w as treated and admitted to the burn unit.
