Investigation Summary
On October 18, 2000, an employee was operating a Crown Stand-up Reach Truck, Model Number 45RTT-S, Serial Number 1A11836, preparing to install a charged battery. The employee was driving in reverse when he noticed that he was going to hit a yellow upright concrete pole. He lifted his left foot off the brake pedal to stop the reach truck and stuck his left foot outside the running lines of the reach truck. The employee's foot was caught between the pole and forklift. He suffered a compound fracture which eventually resulted in a partial amputation of his left leg. The employee was hospitalized and treated for his injuries.
