Investigation Summary
On November 26, 1996, Employee #1, an employee of an employee leasing company assigned to work for a plumbing company as office staff, was injured when she fell through a roof. On the day of the accident, she went to a business site to conduct paperwork transactions with owner of the plumbing company. The owner of the company stated he had talked to Employee #1 and completed the business at hand. Employee #1 left his presence, and apparently went up to the attic to help find a water leak. Employee #1 fell through the roof sustaining serious head injury. Employee #1 was hospitalized and treated for a concussion.
