Accident Investigation Summary
Summary Nr: 201101292Event: 12/13/1999Employee Injured When Finger Amputated
This employer grows cut stock floral products and flowers. Employee #1 works as the manager of one of the farms. Employee #1 was checking the size of the opening on a Gandy soil amendment applicator. Employee #1 did this with the machine running. Employee #1 reached into the bottom of the machine and placed his finger in one of the openings for the material to flow out. Employee #1's son, at the same time, closed the slide that adjusts the opening. Employee #1's ring finger on his left hand was amputated up to the first joint.
