At 9:45 p.m. on June 2, 2017, an employee was loading and organizing items on a pallet situated flat on the ground. The employee caught her left foot in betwee n the pallet slats as she stepped off and fell to the ground, striking her lift hip as she landed. The employee fractured her left hip and was hospitalized.
