At 5:00 p.m. on July 13, 2017, an employee was standing behind the Skylift MDS 6000 while other employees were attempting to remove the bucket. The flagman noticed the skylift was tipping over to the right when he informed the operator to scope out. The employee ran from the back of the skylift and in the path of the tipping skylift striking and killing the employee.
