Accident Investigation Summary
Summary Nr: 96688.015Event: 06/21/2017Employee Falls Off Platform And Receives Multiple Injuries I
At approximately 9:00 a.m. on June 21, 2017, Employee #1 was unwinding the butyl tape as the other employee applied it to the structure. She stepped off the pla tform, falling and striking the ground and tin siding. Employee #1 suffered frac tured clavicles, ribs and vertebrae, injured spinal cord and lacerated head in t he fall. Employee #1 was transported to a medical center, where she received tre atment and was then hospitalized.
