Employee crushed to death by 3,320 lb pipe

On February 15, 1994, employees working for Carl E. Smith were loading steel flume pipe and swamp mats onto semi-trailers. The flume pipe is 20 and 30 inches in diameter and 40 feet long with 3/8 inch wall thickness. The 20 inch diameter pipe weighs approximately 3,320 pounds. While the employees were loading a 20 inch diameter pipe with an end-loader lift onto the truck, the pipe rolled off the truck and fell on Employee #1. He died. The employer was cited for four serious violations.
