At 1:30 p.m. on Mary 14, 2017, an employee was working with a coworker and was adjusting some machinery. The employee reached for a piece of string that was in a box while their coworker activated the machine and struck the employee's fingertip. The employee amputated the tip of their finger. The initial report did not identify which fingertip was amputated, how it was amputated, or what equipment was involved; only that a fingertip had been amputated.
