Summary Nr: 93630.015 Event: 02/21/2017Employee Is Crushed And Killed By Falling Material In Wareho
At 11:30 a.m. on February 21, 2017, an employee was using a forklift to unload g ypsum board and take it to the warehouse. The employee built a single stack comp rised of 12 lifts of sheetrock. After the first stack was built, the employee b egan to build a second stack. When the employee attempted to stack the fourth li ft onto the second stack, several lifts from the top of the first stack toppled over onto the cab of the forklift. The overhead guard and the employee was crush ed. The employee was killed.
