Investigation Summary
At approximately 10:50 a.m. on December 13, 1993, Employee #1 was attempting to light the pilot on a chicken fryer with a hand-held lighter, when a flame shot out from the pilot area causing flash burns on her cheek and right wrist. Employee #1 was transported to Carroll County General Hospital, where she was treated for 1st-degree burns and released the same day. Employee #1 stated that the previous supervisor had taught her how to light the fryer's pilot light. Total Mechanical Services sent a mechanic to inspect the gas fryer. After complete diagnostic testing, the fryer was found to be working OK.
