Accident Investigation Summary
Summary Nr: 79316.015Event: 09/03/2015Employee Is Killed In Fall Through Unguarded Shaft
At approximately 11:30 a.m. on September 3, 2015, an employee, an electrician, w as on the fourth floor standing next to a ventilation shaft observing a coworker , a foreman, take measurements to determine where to locate an electrical racewa y on the roof. After he had taken measurements, the coworker turned around and t ook a couple of steps to go back to the roof with the employee. When he heard t he employee scream, the coworker looked into an unguarded 23.5-inch wall opening on ventilation shaft and the employee had already landed at the bottom of the s haft. The employee received head and body injuries and was killed.
