At approximately 11:15 a.m. on June 2, 2015, Employee #1 was painting the exteri or of a two story building. He placed a step ladder on a tiled roof top. The la dder was unsecured and Employee #1 slid off of roof approximately 20 feet landin g on his head. Employee #1 was killed.
