Summary Nr: 14502991Event: 04/06/1994Employee Suffocates In Hopper Of Beans
Employee #1 had apparently climbed on top of a hopper while beans were being unl oaded, but no one saw him there. About 15 minutes later, the beans plugged up an d coworkers saw a shoe in the hopper. They tried to unload the hopper quickly an d retrieve Employee #1, but he had suffocated.
