Summary Nr: 69596.015Event: 12/17/2014Employee Crashes Truck Into Warehouse Wall And Is Killed
At 6:30 a.m. on December 17, 2014, Employee #1 was working inside of the warehou se, preparing to load a 2014 Ford E350 16 foot box truck, VIN# 1FDWE3FL7EDA55684 . Employee #2, the truck driver, walked to the parking lot located on the north side of the building, then south to the fence line of the parking lot. Employe e #2 got into the truck, and with the vehicle still in drive, Employee #2 presse d his right foot on the brake petal and reached across the cab of the truck to r oll down the passenger side window. Employee #2's right foot slipped off of th e brake petal, onto the accelerator, which caused the truck to travel south 47 f eet to the 24 foot wide by 12 foot tall North Bay rollup door that was in the cl osed position. The truck continued through the rollup door into the warehouse s triking insulation material, a 4 foot by 5 foot fabricated framed scaffold, a ce ment ballad and Employee #1. The truck came to a stop when it crashed into the w arehouse bathroom wall which was 139 feet from the North Bay rollup door. Employ ee #1 was struck and killed by the truck and Employee #2 received multiple blunt force injuries.
