At 2:00 p.m. on October 06, 2017,an employee was performing his daily assigned task of cleaning a rotary press and rollers. In performance of this activity, the machine's rollers were energized (rotating) so that the employee was able to wipe down the rotating rollers with a cotton cloth containing a solvent to remove printing residue from the prior production run. As the employee was wiping the chrome roller with his left hand, he inadvertently selected operation mode with his right hand. This caused the machine's chrome roller to move closer to the dip roller and narrowing the incoming nip point. As a result, the employees' left index finger was pulled into the gap between the rolls and was amputated. The employee was hospitalized and received treatment for his injury.
