At 1:40 a.m. on September 1, 2017, a maintenance technician was repairing a broken pin on a automatic tire balancer. LOTO procedures had not been applied to the machine. The employee opened the gate which had safety electrical interlocks installed. The top chuck is spring loaded. One spring broke and the chuck fell onto their finger. Stored energy of the top chuck with the spring and gravity had not been dissipated. The employees proceeded to remove the bolts that held the guard over the drive belt. The employee reached out and touched the belt to ensure it had stopped. The belt was still moving and snagged the glove. This pulled the employees fingers into the ingoing nip point between the belt and the pulley.
