At 6:15 a.m. on August 26, 2017, Employee #1 was driving a forklift. The forklift began to slide and the employee stuck his foot out of the forklift to brace himself. Employee #1's foot became caught between the forklift frame and the metal guard barrier (bollard). The employee sustained a broken foot and was hospitalized for his injury.
