At 9:45 a.m. on August 28, 2017, Employee #1, a material handler, was assisting a coworker in the area of the shredder. The employee noticed that the cardboard bin under the shredders cyclone was full and the plastic had started to back up into the discharge chute of the cyclone. The coworker used a forklift to remov e the full bin and Employee #1 placed an empty bin under the cyclone/discharge c hute. Because very little plastic was discharging from the cyclone, the employee reached into the discharge chute in an attempt to free any plastic that was clo gging the chute. Employee #1s hand became caught in the rotary airlock and he s ustained a partial finger amputation to his right first finger and crushing inju ries to his right hand. The employee was not hospitalized.
