At 8:30 a.m. on July 7, 2017, Employee #1, employed by a concrete construction company, was pouring concrete at a paving jobsite. While using a wrench to adjust a belt on the paving machine, the machine surged. Employee #1's right arm was pulled into the conveyor. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for the amputation of his arm from the elbow down. | |||||||
