At approximately 10:30 a.m. on June 30, 2017, an employee was beginning to repai r a leaking pneumatic fitting of an abrasive blasting machine. He shut off blast ing machine and disconnected the media hose before walking to another building t o get a wrench to remove the worn fitting. While the employee was away from the machine, a coworker turned the machine back on and walked back to his work area . When the employee returned and was lying in front of the fitting, the coworke r triggered the valve to open. The employee was injured when the machine was not locked out and it re-energized. The employee was hospitalized and treated for h is injuries.
