At 8:15 p.m. on December 6, 2016, a contracted driver was attempting to attach a trailer tandem to the trailer attached to his cab. As the contracted driver backed into the tandem trailer, the tandem trailer pushed into an empty trailer body that was parked perpendicular. The empty trailer moved, and crushed another driver, Employee #1, between two trailer bodies. Employee #1 was crushed and killed.
