At 6:00 p.m. on November 28, 2016, an employee had climbed a power pole and was waiting to receive materials from below to replace the pole top insulator. While the employee was waiting to receive the materials, a strong gust of wind snapped a dead tree. Part of the tree fell and struck the employee in the head, resulting in a fatal head injury. The employee was wearing a hard hat, electrical insulating gloves, leather gloves, FR clothing, climbing gear and Cinch-lok pole climbing device.
