At 9:00 a.m. on March 4, 2015, an employee, maintenance manager was assisting a coworker, forklift operator, in the placement of a large oxygen compressed gas c ylinder into a metal bin for the purpose of transporting this bin to another loc ation at the work site. As the cylinder was being guided into position, the empl oyee was struck in the head by the forks of a suspended load while helping the f orklift operator move a large oxygen cylinder. The forks came down striking the employee in the head and he was killed from blunt force trauma to the head.
