At 1:00 p.m. on January 6, 2015, Employee #1, employed by a construction contractor at a multi-employer construction site, was working from the platform of a scissor lift. According to coworker, Employee #1 answered a call from their supervisor for finishers. Employee #1 put on his gloves, grabbed a mud pan and knife, and bent over the lift guardrail to reach the area he was finishing. Employee #1 fell over the guardrail and to the ground below, a fall height of approximately 12 feet 6 inches. Emergency services were called, and Employee #1 was transported to a hospital. He was admitted and treated for head trauma. Employee #1 died from his fall injuries. This fatality was investigated by Arizona OSHA.
