At 8:45 a. m. on December 11, 2014, Employee #1 was transferring RC-250, a flammable liquid, from the tank farm into his truck. Once the employee was finished transferring the RC-250, he noticed that the line he was using had become stuck due to the cold temperature. Employee #1 decided to use a propane torch to unfreeze the line. Once he lit the propane torch, the flammable liquid ignited, causing an explosion. The employee sustained burns to his body and was killed.
