At approximately 9:00 a.m. on June 6, 2012, an employee was walking outside a pr otective fermentation-box rail, while performing sanitation duties. The employe e saw a puddle of water inside the rail, and went under it to access the water w ith a floor mop. Although the machine was moving slowly, the employee was struc k by a bread trough or basket that carried fermenting bread dough. He was pinned at the back rail until the machine could be stopped. The employee was hospitali zed for severe pain in the legs and hip areas, although no broken bones were ide ntified by X-Rays. On July 15, 2012, approximately six weeks later, employee d ied while sleeping. According to coroner report, the cause of death was overdose of drugs and alcohol, thus deemed not work-related.
