At 10:00 a.m. on July 17,2012, Employee #1 was descending on a ladder to go to t he main floor. Employees had placed an extension ladder at the base of the north ern end of a floor hole. The hole was the opening for the stairs going to the ba sement level. Employee #1, who was facing the north as he was descending the lad der, stepped down off the ladder to the south, where the floor hole was located and fell 10 feet on to his back. Employee #1 was killed due to trauma from the f all.
