At approximately 4:30 p.m. on June 26, 2012, a die place crew, which included an employee, three coworkers and one crane operator, had just taken a die from pre ss area B19 and set it into the die storage area. The employee walked over to th e next die to be hooked up to the cab crane. The employee was holding the lifti ng hook of the crane in one hand and had the sling, which was still attached to the lifting hook, in the other hand. The sling was approximately 3-ft long and w as low to the ground. The overhead crane was in motion and during this time, the sling got caught or hooked around the outer part of the die. The employee was n ot able to release the sling off the die, and the crane pulled the die, weighing approximately 41,800 pounds, into another die, pinning the employee between two dies. The employee was killed as a result of crushing injuries to his torso.
