At approximately 3:00 p.m. on November 29, 2012, Employee #1, an installer, for a mobile electronics retailer was cutting a piece of ABS plastic with a table saw (Model Number MFR: Rigid, M/N: TS3650, Serial Number: 054289748) when he noticed that the back side of the plastic started lifting and bouncing as he ran it through the saw. The employee reached over the hood guard of the saw with his left hand to straighten the motion of the plastic, his finger came in contact with the saw blade. Employee #1's finger on the left hand was amputated. The employee was taken to Sherman Oaks Hospital, where he was treated for the middle finger amputation and released.
