At 11:25 a.m. on February 24, 2012, Employee#1, a 55-yr-old male public school groundskeeper, was trimming trees while aloft from an extension ladder. While performing the operation, his foot slipped off a branch and he fell approximately 10-11 ft to the concrete below. The employee was not tied-off as required by OSHA standards. He was hospitalized with unspecified fractures.
