On March 18, 2009, Employee #1 and a coworker were coming from different directions on two Crown Pallet Jacks, Number 151(Model Number PE4500-80 and Serial Number 2896 EW-F-POCR) and Number 104 ( Model Number PE3540-80 and Serial Number 6A174624). Employee #1 entered the main aisle without properly slowing down and sounding his horn. He failed to yield the right away to approaching traffic. The two pallet jacks collided, and Employee #1 was thrown off of the pallet jack with his feet striking one of the storage racks. His feet were caught under the racks, and he was hospitalized with two fractured feet.
