On April 24, 2012, Employee #1 was working at a facility at which springs and metal stampings were manufactured. Employee #1 was not a contract employee. The facility was under the supervision of the owner. Employee #1 was operating a Besly automated spring grinding machine, with Serial Number 6257352014. The point of operation guard had been removed, thus exposing employees to the possibility of a serious point of operation injury. Employee #1 was attempting to tighten a nut while the spring grinding jig was rotating. The wrench slipped off of the nut, and the unexpected momentum caused Employee #1 to come into contact inadvertently with the grinding wheel at the point of operation. He sustained an amputation injury of the index finger on his right hand.
