At approximately 11:50 a.m. on June 3, 2008, Employee #1, a laborer, was helping a crew of four coworkers install metal shoring sheets 2-ft by 17ft by 6 ft, weighing approximately 375 pounds into ground. Employee #1 entered the excavation and was guiding the sheet at the bottom while another coworker was standing on top of the excavation. The sheeting was rigged with chains through the hole and hook at the end lip of the sheet. The other end was connected to the backhoe bucket. Unexpectedly, the rigging slipped off causing the sheet to fall on Employee #1 inside the excavation. Employee #1 sustained unspecified fractures, but was not hospitalized.
