On February 2, 2009, Employee #1, of Butte Steel & Fabrication, Inc. was cleaning up the southeast part of the metal roof. While he was cleaning the roof, he fell through an opening in the deck twenty feet to the concrete floor. Employee #1 was hospitalized and treated for broken bones, internal injuries and head trauma.
