On May 22, 2007, Employee #1 was operating a concrete conveyor. He was placing 6-in. inside diameter flexible hose into a wall panel. The hose on the discharge side of a pumping system, that was returning bentonite slurry to a holding tank, separated from a corrugated coupler. The free end of the hose struck Employee #1 and he was hospitalized with severe fractures to his head.
