On February 7, 2012, Employee #1 was operating a Milwaukee Model Number 6190 heavy duty chop saw to cut 1.5-inch polyethylene pipe. Employee #1 was operating the saw with his left hand. He reached with his right hand to retrieve the cut end and the piece kicked up, amputating his right index finger at the distal interphalangeal joint.
