On May 19, 2008, Employee #1 was standing on the third step of a 4-ft ladder to monitor the flow of oranges on a conveyor. He lost his balance, fell towards the conveyor, and arrested his fall with his right hand on the conveyor frame. His left upper extremity was pulled into the in-running nip point of the belt and unguarded head pulley up to his biceps. Coworkers unsuccessfully attempted to stop the conveyor; it was eventually shut down at the central control panel. A supervisor cut the belt to extricate Employee #1. He was assessed at Delano Hospital, and then transferred to the Buncke Clinic, where he was admitted for wrist and finger fractures and left arm and hand abrasions.
