At approximately 9:30 a.m. on June 12, 2006, Employee #1, an iron worker, was operating a JLG commander scissor lift, Model Number 80770505, on an open street rail transport platform 34 inches above grade. The scissor lift was extended about 29 to 32 inches, putting the top of Employee #1's hard hat about 14 feet 10 inches above grade. After a discussion with the safety person, Employee #1 moved the scissor lift from the existing panel to another panel through the use of poorly marked controls. The scissor lift dropped off the platform and ejected Employee #1 into traffic. Employee #1 was knocked unconscious. Emergency services were contacted and Employee #1 when taken to San Francisco General Hospital, where he was hospitalized and treated for a skull fracture to his right occipital lobe and right cingular gyris, as well a cocygeal fracture and abrasion on his left index finger. | |||||||
