At approximately 7:00 p.m. on October 29, 2012, Employee #1, of UPS, was to repairing a conveyor belt after a package had been stuck while in operation. The employee stuck his hand in the conveyor to try to dislodge the package. Employee #1's ring finger met with the point of operation and he sustained a serious injury to his left ring finger. Employee #1 was taken to Palomar Medical Hospital, where his ring finger was amputation. The employee was later released.
