On May 5, 2009, Employee #1, a truck driver, was assisting a coworker, a mechanic, to install and inflate a 14 ply truck tire onto a single piece rim. The tire and rim were placed on the concrete floor, and oil was used as a lubricant. No containment cage was used, and no clip on air tool was in used. While inflating, the tire was partially blown off the rim, and the rim and tire were propelled upward, rapidly. The employee was struck in the head and suffered severe head and neck injuries. Employee #1 died a few hours after the accident, at a hospital.
