On October 30, 2009, Employee #1 was working from a mechanical mezzanine. He was staging sheetrock in the work area. A 4-ft by 8-ft sheet of 0.625-in. plywood covered a hole in the floor. The plywood was not secured to prevent displacement, nor was it color-coded or marked with "hole" or "cover" to provide warning of the hazard. Employee #1 fell 14 ft through the hole to the floor below. He was killed.
