On January 27, 2011, an employee was working for Sasaki Associates, Inc., a firm that provided architectural services. The employee was using a hand-fed circular rip saw without guards, a spreader, or anti-kickback fingers or dogs. The 0.625-inch-thick piece of plywood he was cutting kicked back, causing his left hand to slip onto the blade. He sustained a laceration to his hand.
