On December 9, 2008, Employee #1, a mechanic, was working at a retail tire dealership. While he was filling a tire with air, the tire rim blew out of the tire and struck Employee #1 in the head. He was taken to a nearby hospital, where he died due to a serious head injury. The incident investigation revealed that a large amount of bead lube was present on the inside of the tire being mounted and on the floor in the area where Employee #1 was working.
